Online Services

Welcome to Alexander House Surgery

Alexander House Surgery is a doctors surgery in Farnborough which provides the highest quality healthcare to patients in the area. Our doctors, nurses and all our other staff are dedicated to offering a professional service and this surgery website will help us to keep all our patients up to date with news and information about our practice. We will keep it current with any developments or other details that we feel are relevant to you.

As well as a source of information about our practice, the staff and the services we provide, we hope you will use this website as a useful resource for other health-related information. Please have a look around and do send us some feedback if you like. We can use your ideas to improve our online services and further develop the content of this site.

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up

Patient Online Access

As from 1st April 2016, patients can make appointments, order repeat prescriptions and now also apply to view their medical record online.

You will need to fill in a short form at our reception desk and bring proof of your identity - 2 forms of documentation must be provided, one of which must contain a photograph. Acceptable documents include passports, photo driving licences and bank statements.

It is important that before you apply you are confident that nobody else in your household will be able to access your medical record against your wishes. You may give your consent to someone else to have proxy access, for example relatives or carers, by requesting a Proxy Access to Medical Records registration form at our reception desk.

Proxy access allows parents, family members, carers, care home staff, to have access to their patients’ online records, in order to book appointments, request repeat medication and where applicable, view their medical record. The patient could be 9 or 90 and in need of some assistance in managing their healthcare.

* If a patient U16 has a proxy, access is restricted at 11 and stopped at 16.

Named Accountable GP for all Patients

All current and new patients registered with this Practice are allocated a named GP. If you would like to express a preference as to which GP you are assigned the Practice will make reasonable efforts to accomodate this request. Having a named GP does not prevent you from seeing any other doctor within the Practice.

If you wish to be told the name of your Accountable GP please ask the receptionist when you are next in the Surgery. PLEASE NOTE: there is no need to telephone the Practice for this information, our phone lines are very busy and primarily are for patients with urgent needs.

Take our Friends and Family Test (FFT)

The NHS wants to ensure that you have the best possible experience of care. The Friends and Family Test is a way of gathering your feedback about this experience and helping to drive improvement in GP services.

When you receive care at the Surgery you will be given the opportunity to give your feedback by answering a simple question about your experience.

The results will provide a way for you to easily compare NHS Healthcare Providers so that you know where you and your family can get the best possible care.

The information will also give the NHS invaluable information on what patients think of services, which can be used to help make improvements if required.

Just click here to give us your feedback - Thank you!

For results of feedback given by patients who have already completed this survey click here to see our Friends & Family survey results.
