Alexander House Surgery provides a private Travel Advice and Vaccinations service to our patients.  Advice can be given on travel anywhere for business or pleasure.

To find out what vaccinations, if any, you will need please contact us at least 6 weeks before you travel in order to complete a travel questionnaire (one form for each traveller) and arrange an appointment with our Practice Nurse.

Please give exact details of your destination and return the form/s to reception. At this time you should also make an appointment to see the Practice Nurse in her Travel Clinic.  Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.  The form/s will be assessed by the Practice Nurse prior to your appointment and she will indicate the recommended vaccines and fees.

IMPORTANT INFORMATION

There is a fee for most immunisations. This is payable in full at your first appointment so ensure that you bring payment with you.

WE ONLY ACCEPT CASH OR CHEQUES - SORRY NO CARDS

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.

Any patient not registered at this practice will be charged privately. Costs are available on request.

Patients under 16 years of age require a parent's signature
